PLEASE BE AWARE FROM THE 1ST APRIL 2025, WE WILL NO LONGER BE ACCEPTING/PRINTING PAPER REGISTRATIONS. YOU WILL BE REQUIRED TO REGISTER ADULT/CHILDREN REGISTRATIONS ONLINE. YOU DO NOT REQUIRE ID TO DO THIS. IF YOU ATTEND THE SURGERY TO REGISTER, YOU WILL BE RE-DIRECTED TO OUR WEBSITE.
We are currently accepting new patients in the CM8 postcode. To register at our practice we do require a completed questionnaire found below or at the surgery reception. To register we only ask for one form of ID/Proof of address. This can be anything recent as long as it provides confirmation that the person is yourself. If you are not able to provide this, please speak to surgery.
You are welcome to bring a registration form on behalf of someone else, however ID still needs to be provided and they must sign and complete the form fully themselves, otherwise you may need to attend again.
Please be aware our current registration turn around is 10 WORKING DAYS, if we can get you on quicker we aim to do so.
Please make sure to read the forms carefully, filling in all sections and informing us of any current repeat medication. It is important that this is done correctly so that we can register you quickly and efficiently. We ask patients to get a 1 month repeat prescription from their previous surgery, as this allows us to process your registration our end and add any repeat medication back on to your record. You can ask your surgery to send the prescription to a local pharmacy if you are unable to travel back to your old surgery/pharmacy.

Accountable GP
It is a requirement of NHS England that every patient is allocated an accountable GP. This is to reassure patients that they have a named accountable GP who is responsible for their care. This does not mean that you can only see your accountable GP, you may book an appointment with the GP of your choice subject to an appointment availability.
